Strategic Plan Outcome: O2.Every Student Thrives Socially and Emotionally
Strategic Plan Measure: % of students reporting they feel safe, welcome, and accepted at school.
School “SMART” Goal (Specific, Measurable, Achievable, Relevant, Timely): By the end of the 2023-2024 school year, at least 80% of Five Oaks students will report feeling safe, welcome, and accepted at school.
Strategic Plan Strategy:
Create school-wide structures that intentionally build safe and inclusive learning environments.
- Action: Community Connections
- Action: We meet in school wide advisory twice a week. Advisory students stay in the same cohort for the duration of their time at FOMS and teachers loop with their advisory students.
- Action: Community Events
- Action: BHW team meetings weekly to discuss student data around social emotional, academic, and attendance.
- Action: Kid chats with teams of teachers and counselors. Teachers will complete a form requesting support with a student, their team will convene to look at data and come up with a plan to support the students needs.

Strategic Plan Outcome: O1. All students demonstrate progress toward, and achievement, of the standards
Strategic Plan Measure: % of students in grades 3-8 demonstrating proficiency on the state ELA test
School “SMART” Goal (Specific, Measurable, Achievable, Relevant, Timely):
All students will increase by at least one level on their OSAS ELA test by the end of the school year. Students who have exceeded on the state test will maintain their level of proficiency

Strategic Plan Strategy: Learning targets and success criteria (rubrics) directing all instruction and assessment in all content and courses at all grade levels
- Action: Using outcomes in Canvas to show students their progress on a rubric. Using outcomes in Synergy so that parents are aware of progress as well.
- Action: “Non-negotiables” including first five strategies and explicit teaching of EQ and learning targets
- Action: Academic Coaching for identified students with direct instruction and Lexia intervention.
